02/16/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. A patient was observed to have a painful, stage 3 pressure wound from sitting and laying on a catheter line. The entire bottom of the resident was noted to have scattered, widespread, open areas, with dark red, purplish demarcation where the catheter tubing laid. There was no previous documentation in this individuals’ files that the wound was identified before the health surveyor-requested assessment.
02/16/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. A resident with a history of multiple falls who was a continued fall risk was left unattended and found standing up in front of their wheelchair. After several minutes, an aide heard the resident’s wheelchair fall alarm sounding with a low tone. His care plan didn’t address his supervision for fall prevention, but staff stated otherwise, that he shouldn’t be unattended.
02/16/2018 Failure to provide enough food/fluids to maintain a resident’s health. Multiple discrepancies in records have led to inaccurate assessments of nutritional needs. He has lost weight over several months according to the information on file – but there are questions in accuracy and no follow up was made.

The facility has a communication board system for working with these residents, however, on two occasions while the observer was present, the board was either not used or was found ineffective— leaving the residents frustrated or confused.
