Lexington of Schaumburg is a larger, 214 bed “for profit” nursing care facility offering private and semi-private rooms, post-acute and medically complex care, multiple rehabilitation and therapeutic modalities, and comforts like an on-site beauty shop, private bathrooms, and cable television on flat screen TVs. 10/19/2017 Failure to protect each resident from all abuse, physical punishment, and being separated from others. A resident was treated roughly when being taken to the bathroom early in the morning and was slapped repeatedly around the shoulders and neck. The resident reported that the CNA was angry with her for being too slow, saying that she had a lot of other people to take care of. The resident felt as if the CNA was still mad at her for reporting the incident, and was visibly upset when describing her feelings. The administration did very little in follow up, disciplinary action, and showed little concern for the resident’s well-being.

10/19/2017 Failure to keep the rate of medication errors (wrong drug, wrong dose, wrong time) to less than 5%. Multiple observances were made of haphazard medication administration, with an error rate of 23%. Nebulizer medicine was still visible in the collection chamber when the RN determined that the treatment was “done” due to time. The medication orders were to finish the dose, not follow a clock. Another situation involved IV medication. The nurse wasted over half of the medication trying to eliminate bubbles in an improper fashion and proceeded to rush the infusion process. It is indicated that this medication should take about 30 minutes to infuse, it took roughly 7. Another involves incorrect administration of injectable insulin. The insulin was not fully or accurately administered, with no way to tell if the full dose was injected.

10/19/2017 Failure to have a program that investigates, controls and keeps infection from spreading. Multiple instances of improper hand hygiene and glove use were observed. Some examples; One RN took a call on their cell phone and proceeded to prepare and administer injectable medications without any hand hygiene or use of gloves. A CNA didn’t change gloves after moving a trash can, cleaning a resident’s bottom, and moving the trash can again, and finishing incontinence care.
