Lexington of Elmhurst is a 142 bed “for profit” nursing home and health care facility in Elmhurst, IL. They offer private and semi-private rooms, private bathrooms, comprehensive wound care and post-acute care units, full rehabilitative services, and multiple therapeutic modalities. Their web advertising states that they are “passionate about offering superior health care and senior living options,” however, recent health inspections show serious medication errors, negligent incontinence and pressure sore treatment among others;
03/14/2018 Failure to ensure that residents are free from significant medication errors. A resident was hospitalized with multiple issues including lactic acidosis as a result of multiple medication errors, including missed doses and medicines administered at the wrong times - there should be no missed doses or changes in timing of administering these particular medicines, as well as thorough documentation of fluid intake and output when on this medicine.
05/02/2018 Failure to Provide care and assistance to perform activities of daily living for any resident who is unable. Multiple residents were observed to have been left in their rooms or in their wheelchairs, without interaction, and without incontinent care for hours at a time. When provided care after 1:15pm, each had a soaked incontinence brief and one also had a large amount of loose stool present.
05/02/2018 Failure to provide enough nursing staff every day to meet the needs of every resident; and have a licensed nurse in charge on each shift. Multiple instances of insufficient staffing were observed. There simply wasn’t enough staff on hand to properly care of all of the residents basic needs, such as incontinence care, repositioning care to prevent pressure sores, and making sure food got to residents in a timely manner and edible state. Resident council notes state that residents had missed rehabilitative care sessions because that staff had been pulled away to work on the care floor. Complaints were made that the facility was chronically understaffed and the CNAs that hadn’t quit were sitting around complaining instead of providing care.What You can do to Help
