4/12/18 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Staff failed to provide adequate repositioning for one resident which resulted in an avoidable coccyx pressure ulcer.
4/12/18 Failure to provide enough food/fluids to maintain a resident’s health. Staff failed to provide assistance with eating to two residents and neglected to address the weight loss of these and a third resident.
4/12/18 Failure to provide and implement an infection prevention and control program. A staff member failed to put on gloves and protective gown while in a resident’s room who was under contact isolation for a medical condition, increasing the risk of spreading an infection.Protecting Senior Residents
