Aperion Care Elgin is a 102-bed nursing home facility in Elgin, IL. This facility offers post-hospital care and rehabilitation with physical, occupational, and speech therapies available 7 days a week, with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.” Aperion Care Elgin has had 20 complaints in the past 3 years that have resulted in a citation and is rated by medicare.gov as “below average” for staffing, health inspections, and fire safety.
05/10/2018 Failure to immediately tell the resident, the resident’s doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident . A resident passed away after experiencing a medical event that was not immediately reported to a physician. The change in condition was noticed by staff, and no action was taken until too late.
Two CNAs provided care for the resident in the morning, one instructed the other to put the resident back to bed after her shower as she wasn’t feeling well. The other noted that the resident was pale, seemed tired and head lobbed back repeatedly while showering her. Later, she was taken to the dining room for lunch.
Another resident reported that they tried to alert a staff member that the resident was having a medical event (the actual medical outcome is redacted in the public file). The alerting resident stated that he was told to “mind his own business, she’s fine.” This resident was upset that he was brushed off, especially since he had experienced that condition twice before. The resident then passed out in the dining room. She was removed to her room where they performed CPR until an ambulance arrived. She passed away at the hospital.
05/10/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. Multiple residents experienced falls or painful, dangerous transfers. One resident was found lying awkwardly across the bed, with his feet touching the ground on a fall mat. Multiple housekeeping staff member entered the room, left, and didn’t notify nursing staff of the residents’ condition.
The surveyor alerted the RN, who checked on him. The resident was attempting to go to the bathroom – he is supposed to use a call light for this. The resident has had 2 falls in the past three months and is listed as a high fall risk. Another resident was lifted incorrectly to transfer, resulting in painful pressure on their broken ribs.
05/10/2018 Failure to provide appropriate care for residents who are continent or incontinent of bowel/bladder, appropriate catheter care, and appropriate care to prevent urinary tract infections. A resident was observed at 1:35pm with a soaked incontinence brief that had leaked through to the resident’s pants. The CNA stated she last changed his brief at 7:00am. When care was finally provided, the CNA failed to cleanse the resident’s bottom before putting on a new brief.
03/20/2018 Failure to provide enough nursing staff every day to meet the needs of every resident; and have a licensed nurse in charge on each shift. Extremely short staffing led to lacking care of residents needing extensive help with bathroom needs. At times, there was only one CNA on the floor to help 90 residents.
A resident was so exasperated with being in her overly soaked incontinence brief, she crawled into the hallway in search of a nurse. She stated that call lights are ineffective, no one comes. Records confirm that on all of the dates reviewed, care staffing was extremely short, with a maximum of 4 CNAs on schedule to care for 90 residents, many of whom are completely dependent on others for care.
