Heritage Health Normal is a large facility with 141 beds that offers long-term and intermediate nursing care through onsite physical, occupational, speech, and respiratory therapies. The facility also features an Alzheimer’s care unit.

12/06/2018 Failure to provide and implement an infection prevention and control program. Staff failed to wash hands after providing catheter care and a dressing change for a resident. A CNA removed the gloves used during urinary catheter care on a resident but did not perform hand washing before entering another resident’s room to provide care there. A nurse then came to perform pressure ulcer treatment on the first resident and also failed to wash hands after going to another room.

12/06/2018 Failure to keep residents' personal and medical records private and confidential. The facility failed to provide personal privacy during wound care treatment for a resident. A nurse and CNA failed to use the privacy curtain while performing pressure ulcer care for a resident, leaving the resident’s buttocks viewable to the resident’s roommate and people in the hallway.

12/06/2018 Failure to provide safe and appropriate respiratory care for a resident when needed. The facility failed to maintain and provide necessary respiratory equipment for one resident. The resident uses an oxygen concentrator machine. It was observed that the nasal cannula tubing and humidifier bottle which need changed weekly were not labeled or dated. It was also discovered that the wrong tubing was being used.

Protecting Nursing Home Residents from Abuse

Using the ratings and health inspection reports on medicare.org should give you a good idea of what nursing homes to consider when you are shopping around for assisted living services. Additionally, you can also ask acquaintances, medical professionals, and local government agencies for recommendations or additional details about nursing homes in your area.
