Aperion Care Racked Up $2M in CMS Fines for Nursing Home Violations
Illinois nursing home facilities run by parent company Aperion Care have been hit with roughly $2 million in total fines from the Centers for Medicare & Medicaid Services (CMS) for healthcare deficiencies threatening the safety of their residents.
CMS inspection reports have identified hundreds of care requirement violations across Aperion Care’s homes in and near Illinois.
Which Aperion Care Homes Have Had the Most Significant Violations?
According to data collected by ProPublica, the following Aperion Care facilities had the highest total fines issued and number of significant deficiencies identified by CMS :
- Aperion Care Forest Park: This facility has racked up almost $400,000 in fines across five serious deficiencies and 77 total violations.
- Aperion Care Wilmington: This home has been fined just over $293,000 for 41 total violations, three of which were considered serious.
- Aperion Care West Chicago: This facility received more than $250,000 in CMS fines for 60 total violations, three of which were serious.
- Aperion Care Lakeshore: This facility had 95 total deficiencies identified, more than any other Aperion home — but without any “serious” violations. It has generated nearly $220,000 in fines to date.
- Aperion Care Chicago Heights: This home has received more than $106,000 in total fines for two serious violations and 35 total deficiencies.
The Consequences of Severe or Repeated Violations
While most violations identified by CMS are considered minor and fixable, some represent significant threats to residents’ safety and well-being. Others are issued in response to tangible harm residents have already suffered thanks to substandard care.
Some of the recent “serious” deficiencies identified at Aperion Care facilities include:
- In March 2025, staff at Aperion Care Forest Park failed to properly follow hypoglycemia guidelines, leading to the hospitalization of a diabetic resident. The facility received a $134,843 fine for four violations identified on that complaint report.
- Aperion Care West Chicago staff failed to supervise a resident who was known to pose a risk of leaving the facility. Workers did not realize the resident was missing for six hours, and he was later found dead near the facility, according to a January 2025 complaint report.
