09/01/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. Multiple instances of careless, improper care in transferring residents led to injuries, and in some cases, no follow up care after the fall. Safety precautions for minimizing fall injuries such as having a call light within reach for fall-prone residents were also not followed.

09/01/2017 Failure to keep accurate, complete and organized clinical records on each resident that meet professional standards Documentation errors in reference to a resident's need for assistance in mobility potentially contributed to a fall in which the resident hit her head and was left bruised. This high risk for fall individual had recommendations for fall prevention listed in one area, and recommendations for allowing her to transfer and move about with only the assist of a wheeled walker.
