04/04/2018 Failure to provide safe, appropriate pain management for a resident who requires such services. A new resident was observed in pain, being refused medication, with no documentation of her most recent dose. She was documented to have called for pain medication at 2:25 a.m. and had been refused medication all the way through to 12 noon the following day. According to the data, she did not receive any pain medication until the next day. Her records state to administer pain medication as needed.

03/13/2017 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. Multiple residents experienced falls due to negligent care. One of these residents were cited as needing emergency room care after an avoidable fall; she had been placed in the wrong type of wheelchair and fell out.
