Aperion Care Chicago Heights is a 200-bed “for-profit” nursing home and psychiatric care facility in the South suburbs of Chicago. They are self described as a safe, structured residential environment offering Psychiatrist and Physician supervised care plans for each resident. They offer speech and occupational therapies, as well as pain management, diabetes care, substance abuse recovery programs, and behavior management.

While this sounds great on paper, Levin & Perconti have found some troubling facts about Aperion Care Chicago Heights;

General Patient Care Failures Abound

Since 2015, there have been a total of 18 complaints resulting in health inspection citations. The most recent inspection reports include violations of privacy, medication mishandling, and cleanliness, among others – below is a sampling of the violations listed;

11/14/2017 Failure to keep each resident’s personal and medical records private and confidential

11/15/2017 Failure to safely provide drugs and other similar products which are needed every day and in emergencies, by a licensed pharmacist.

11/15/2017 Failure to provide necessary care and services to maintain highest well being of each resident

11/17/2017 Failure to provide a safe, clean, comfortable and homelike environment

Though their last inspection was in late 2017, a quick glance at the reviews on Google still paint a bleak picture of the care provided at Aperion Care Chicago Heights. What people are saying recently is horrifying:

“Conditions in this facility are appalling”; “stench of urine and mold permeating the rooms”; “she had a rash all over her very dry skin.”

“My aunt has bed sores… she complained about how painful they were”; “The bed she was lying on appeared to be infested with bedbugs.”

“Our grandfather was starving and has lost quite a bit of weight since being there.”

The annual health inspection dated January 27, 2022, led to Aperion Care Chicago Heights receiving citations for the following:

  • Failure to ensure residents are not subjected to physical, verbal, sexual, or psychological abuse, neglect, exploitation, or misappropriation of property
  • Failure to provide adequate supervision for three residents

As a result of these violations, two patients were physically assaulted by other patients, resulting in injuries that led to hospitalizations. One assault victim’s arm was broken, and the other required sutures on her eyebrow. The patient with the broken arm was shoved by another patient, causing her to fall.

The patient had wandered into the other patient’s room and refused to leave when asked. The director and assistant director of nursing told inspectors they were unaware of the patient having a history of wandering, but this information was listed in her care plan, which was updated on May 14, 2020.

The other assault victim was attacked by a patient with known aggressive tendencies who had displayed increased agitation and aggression leading up to the incident. Aperion Care Chicago Heights was fined $2,200, double the usual amount, due to the facility’s violation of codes designated as high-risk.
