Aperion Care Dolton is a small, 80 bed nursing home and rehabilitative care facility in Dolton, IL. This facility offers post-hospital care and rehabilitation with physical, occupational, and speech therapies available 7 days a week, amenities like a beauty salon and transportation, along with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.”

Aperion Care Dolton is rated “much below average” by medicare.gov for their staffing levels. With just 11 minutes of Registered Nurse time per resident per day (the average being 46 minutes) and no permanent Director of Nursing on staff (the previous DoN quit in March, and as of August 2018 there was still no replacement), it’s easy to see why they are rated so poorly.

06/08/2107 Make sure that residents are safe from serious medication errors. During observation, an LPN was seen administering insulin to a diabetes patient. After taking the glucose reading, they proceeded to administer nearly half of the dose prescribed by the doctor, even after the surveyor questioned them.

08/02/2018 Protect each resident from all types of abuse such as physical, mental, sexual abuse, physical punishment, and neglect by anybody. A resident stated that a caretaker bent her fingers back to pry a remote for their call light out of the residents’ hand. The resident wanted to keep her light on so she could see who was coming in her room and had her bed fully upright.

The LPN wanted her to put the bed lower for safety reasons and went to take the remote from her. The LPN claims the resident didn’t have the remote in her hand when she came in, that it was laying on her chest. The resident later complained of pain and was evaluated. Nothing was dislocated or broken, but some soft tissue swelling was observed.

08/10/2018 Provide appropriate pressure ulcer care and prevent new ulcers from developing. Prevention and proper care measures were not taken resulting in a stage 3 pressure injury on a residents’ heel acquired while in the care of the facility. This resident has a specialized mattress in place intended to reduce pressure and allow for healing of pressure injuries.

Staff had no idea how to operate the mattress controls. It was set to “static,” which meant it was operating as a regular bed. Additional nutritional supplements that were recommended between regular meals to promote healing were not being administered.

08/10/2018 Provide enough food/fluids to maintain a resident’s health. A resident had specific orders to have additional fluids, recommended amount around 2500. According to records, the resident was receiving around 480 – 720ml total. The resident had abnormal labs indicating kidney dysfunction and the fluid pushing was imperative to their health.

08/10/2018 Have a registered nurse on duty 8 hours a day; and select a registered nurse to be the director of nurses on a full-time basis. The acting Director of Nursing is also the Aperion Care regional nursing consultant, with no set regular hours. This individual was at the facility 2-3x a week for 8 hours a day.
