04/21/2018 Failure to Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. The facility failed to provide sufficient supervision for a resident with a high risk for elopement from the facility. During medication distribution around 4am, the resident was missing from his room.

The CNA went looking in usual places around the facility and drove around the area looking for him. He left via his room’s window, where he pushed out the screen. The police were called, and the resident was found on the ground after falling off two bottled water pallets he was sitting on. He had stolen a bottle of alcohol from the pharmacy and drank the whole thing. He was taken to the ER for abrasions to the cheek and chin and an injury to the wrist.

04/21/2018 Failure to provide and implement an infection prevention and control program. Proper infection control measures were not taken when dealing with multiple individuals who had highly transmissible infections present. In one instance, a glucose meter was placed directly onto the bed linens of a MRSA-infected resident, without a barrier of any kind. The nurse carried out the glucose testing, and then placed the machine directly on the medication cart, without a barrier, and without wiping with a sanitizing towel.

Another resident, who was positive for C Difficile (a contagious and not easily treated bowel infection) was observed receiving incontinence care. The resident was observed to have large amounts of liquid stool on their body, gown, and linens. The CNA performing the care did not use protective measures such as a gown, mask, or goggles while providing care. The CNA then went on to assist with passing meal trays in the dining room.

06/14/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Multiple pressure wounds were observed on a resident’s heels and coccyx. The wound physician stated that the resident had developed new pressure sores in multiple locations while at the facility. The physician stated that these pressure sores could have been prevented if the resident was repositioned at least every hour to help with healing. The Director of Nursing confirmed that records indicated the resident was repositioned only every two hours.
