Aperion Care International is a 218 bed “for profit” nursing home facility providing nursing and rehabilitative services on a tree-lined residential neighborhood in Chicago, IL. They offer personalized post-hospital care, individualized plans for care, ensuring medical needs are taken care of while recovering and preparing to return home. Other amenities include bilingual staff, a beauty salon, ice cream parlor, flat-screen TVs in each room, and complimentary Starbucks coffee.

Free strong coffee can’t make up for shortcomings in care that Aperion Care International has suffered in the past few years. Since 2015, this facility has had 36 health violation complaints that have resulted in citations made by the State of Illinois. Here’s a sampling of the violations:

Carelessness, Negligence, Poor Care

03/02/2018 Failure to honor the resident’s right to a dignified existence, self-determination, communication, and to exercise his or her rights. Two CNAs were found to have made a video in a resident’s room, jumping on the bed (with shoes on) and shared it publicly on social media.

03/02/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. The facility failed to take proper measures to prevent pressure ulcers (severe bed sores) from forming, and a result, at least one individual was hospitalized with a pressure ulcer that had formed and deteriorated to a stage 3 within 3 months of being admitted to the facility.

03/02/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. Carelessness on the part of a CNA allowed a resident to fall out of bed and require hospitalization for a laceration to the forehead. The CNA left the resident unsecure and unattended while retrieving a lift to transfer the resident.

06/08/2018 Failure to procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards. During a kitchen tour, several expired food items were found as well as unlabeled food and perishables left on counters. Improper food safety and cleaning procedures were also observed.

Some other violations include failures in infection control (properly sanitizing after cleaning up in a contagious individual’s room), failure to have call lights easily available, and more failures in care for pressure sores.
