Aperion Care Jacksonville is a 113 bed “for profit” nursing home facility in Jacksonville, Illinois. They advertise themselves as a comfortable facility specializing in personalized rehabilitation and post-acute care with physical and occupational therapies available 7 days a week. They even offer a beauty salon, transportation, and pet therapy. Sounds fantastic, right? Levin & Perconti have uncovered some facts on Aperion Care Jacksonville that we feel you should know before you entrust a loved one to their care.

A Long History of Serious Failures in Care

This facility has been flagged by the Department of Health and Human Services as a “Special Focus Facility”, indicating that the facility has an extended history of consistently poor quality of care. It is subject to more frequent inspections. and potentially higher penalties for violations. This facility had a total of 32 complaints reported in the past 3 years, resulting in health inspection violations.

Some of the most recent violations include ignoring resident complaints (and not informing them how to file such a grievance), overflowing urinary drainage bags sitting on the floor of catheterized patients, and endangering the lives of individuals with feeding tubes placed;

5/3/2018 Failure to provide appropriate care for residents who are incontinent of bowel or bladder and provide appropriate catheter care to prevent urinary tract infection. A resident’s urinary drainage bag was repeatedly found to be overfull and resting on the floor.

5/3/2018 Failure to provide appropriate care for a resident with a feeding tube An inspector observed a Nurse’s Aide attending to incontinence needs fully lowering the head of the bed of a resident while they were receiving a pump feeding. (The bed should not be lowered below 30 degrees while a feeding is occuring for safety of the resident, improper feeding procedures can lead to aspiration of food.)

10/31/2017 Failure to give proper treatment to residents with feeding tubes to prevent problems (such as aspiration pneumonia, diarrhea, vomiting, dehydration, metabolic abnormalities, nasal-pharyngeal ulcers) and help restore eating skills, if possible. Over the course of several months, a resident lost a significant amount of weight due to negligent tube feeding practices as well as broken and unattended to equipment.

10/31/2017 Failure to make sure that each resident gets a nutritional and well balanced diet, unless it is not possible to do so. A resident was observed scraping the plate after eating all of their food, and then being removed from the table with no further food on multiple occasions. When asked by the health inspector if they had gotten enough food at breakfast, they responded no.

Unfortunately, the list goes on. It includes unsanitary dishwashing, improper wound care, neglecting to properly care for patients to prevent pressure sores, and more.
