They advertise personalized-post hospital care, individualized care plans, ensuring medical needs are taken care of while recovering. The facility has been recently remodeled and is situated across from a community park.
A shiny, new name and a fresh interior can’t make up for deficiencies in care that Riverwood Rehab has suffered in the past few years. This facility has had 38 complaints in the past three years that have resulted in citations made by the State of Illinois. Here’s a sample of the violations:
11/07/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provide supervision to prevent avoidable accidents. Faulty door alarms and inattention by staff allowed a resident to leave the facility and wander around town, wearing only shorts, a tee shirt, and no shoes, in late October. This led to the resident being taken to the emergency room after being picked up by the police. (The facility later re-evaluated the elopement risk of all residents and trained staff on such matters.)
11/07/2017 Failure to immediately tell the resident, the resident’s doctor and a family member of the resident of situations (injury/decline/room, etc.) that affect the resident. Nearly a week after the elopement incident, neither the residents’ primary physician nor the Medical Director of the facility had any notification that their patient had left the facility unattended.
01/11/2018 Failure to timely report suspected abuse, neglect, or theft and report the results of the investigation to proper authorities. A nurse’s aide slapped a resident in front of two other caretakers, who didn’t report the incident for several days. When it was finally reported, the aide that was witness to the incident said “I felt I needed to report it, because if that is what she would do with 3 other people in the room, what would she do when there was no one in the room with her?”
