Aperion Care of Midlothian is a 91-bed healthcare facility providing short term rehabilitation and mental health services. They offer physical, occupational, and speech therapies 7 days a week, with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.” Other amenities include cable TV, transportation services, activity schedule and beauty salon.
Aperion Care Midlothian is rated “much below average” by Medicare.gov, with below average health inspection reports and 25 complaints that resulted in citations in the past three years. Staffing levels are well below average, with residents receiving half the Registered Nurse, Nurse Aide, and therapy time than the national average, less than an hour of total care time per resident per day. Some of their recent care violations are as follows:
07/13/2018 Failure to make timely report of suspected abuse, neglect, or theft and report the results of the investigation to proper authorities. With known policies of reporting unexplained injuries in place, a record review showed a “new skin concern” to a resident’s lower right wrist with no notification of administration, doctor, of family, and no further investigation made.
07/13/2018 Failure to ensure drugs and biologicals used in the facility are labeled in accordance with currently accepted professional principles; and all drugs and biologicals must be stored in locked compartments, separately locked, compartments for controlled drugs. Upon review of medication carts, the surveyor found multiple expired and unlabeled medications and supplements (no marking made for opening or expiration/discard date.) which if administered could have unknown effects.
07/13/2018 Failure to honor the resident’s right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely. Water damage to multiple areas including in bathrooms and ceilings in resident rooms, water damage to brick exterior, and crumbling drywall were noted.
Ants entering the building through a large crack in the wall. Missing and broken tiles and tub handles. Cracks in the tub, missing shower floor tiles, missing drain cover in shower leaving a dangerous hole for a drain, reports of water backups in the shower. Administration stated they had a period of about 4 months with no maintenance director on staff. No maintenance work orders were on file in regard to the major issues observed.
