Aperion Care Spring Valley is a 98-bed skilled nursing and short-term rehabilitative care facility in Spring Valley, IL. They offer physical, occupational, and speech therapies 7 days a week, with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.” Other amenities include cable TV, WiFi, fenced courtyard, and beauty salon.

07/21/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. Failure to supervise and prevent a fall resulted in a head injury requiring an emergency room visit. The resident was seen slumping/bending far forward in their wheelchair by a staff member who was wheeling another resident to their room.

The CNA told the resident to lean back and that she would help her in a minute. The CNA continued on her way, delivering resident she was pushing to their room, leaving the slumping resident unattended. When she returned, she found the resident on the floor, bleeding from her head. The resident required two staples to close the laceration.

01/12/2018 Failure to ensure medication error rates are not 5 percent or greater.

Multiple medications were left out for a medication pass for a resident for multiple days. The LPN stated that there is a discrepancy in dosage amount for the resident and what is on the cart for one medication, and they were waiting to hear from the doctor before continuing administering. Another medication was also unavailable to give. These medications were missed for at least two days.

02/02/2017 Failure to make sure that each resident who enters the nursing home without a catheter is not given a catheter and receive proper services to prevent urinary tract infections and restore normal bladder function. Improper catheter care was observed while the surveyor was present. The CNA cleaned the resident’s groin area with a cloth and proceeded to wipe the catheter with the same cloth.

While cleaning, the resident’s urine collection bag was held above the level of the resident’s bladder, allowing urine to backflow into the catheter tubing. An antiseptic cloth should be used to wipe the catheter and tubing, and collection bag should always be below the bladder level to prevent infection.
