Aperion Care Bridgeport is a medium-sized nursing home facility in Bridgeport, IL with 99 beds. This facility offers post-hospital care and rehabilitation with physical, occupational, and speech therapies available 7 days a week, amenities like a beauty salon, transportation, cable TV and WiFi, along with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.”

05/25/2017 Failure to have a program that investigates, controls and keeps infection from spreading . Multiple failures in infection control protocol were noted by the surveyor, most notably in an instance of care for an individual who had a high level antibiotic-resistant infection and was to be treated with a high level of infection control.

A team of two CNAs were performing incontinence care, linen changes, and clothes change for the resident. Missed glove changes occurred after taking care of a stool, touching multiple clothing items with contaminated gloves, including the CNA’s own pockets, as well as contaminating a roll of trash bags and a bottle of hand sanitizer before preparing to leave the room with said items.

The surveyor stopped them from leaving the room with the contaminated items. Other issues included the mishandling of a blood glucose test unit and artificial tears container, allowing them to possibly be contaminated.

5/22/2017 Failure to give proper treatment to residents with feeding tubes to prevent problems (such as aspiration pneumonia, diarrhea, vomiting, dehydration, metabolic abnormalities, nasal-pharyngeal ulcers) and help restore eating skills, if possible. An LPN administered 120cc of water through a resident’s feeding tube without checking for proper placement. When questioned, she responded that she doesn’t usually check if she is just giving water. (Tubes should be checked for placement regardless of what is passing through. Failure to do so could lead to a multitude of problems, including aspiration and death.)

06/05/2018 Failure to provide pharmaceutical services to meet the needs of each resident and employ or obtain the services of a licensed pharmacist. The surveyor observed medications being pre-poured, pills placed into cups, each marked with a care with the room number. This is against standard policy and can easily contribute to medication errors.
