Aperion Care Burbank is a small nursing home facility in Burbank, IL with 56 beds. This facility, like most Aperion nursing homes, offers post-hospital care and rehabilitation, with a focus on geriatric psych programs for the needs of dementia and Alzheimer’s residents, along with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.”

In general, Aperion Care Burbank has a good track record of care, with no complaint inspections in the past 3 years. Upon further research we found that their staffing rating is on a decline, with residents receiving only 18 minutes of LPN care each day. This may be reflected in the regular health inspection records, where rushed feeding tube care could present aspiration risks, and mishandled transfers could end in broken bones;

05/25/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. Several residents were placed in risky situations when staff members failed to use appropriate assistive devices for transfers. Residents that require extensive assistance and are unable to stand or transfer themselves independently were seen being transferred without gait belts, picking up by the armpits and moving them.

All the residents that were observed being moved (many unsteadily) had notes on file that they required extensive help for transfers, a gait belt should be used. Moving these residents without assistive devices poses a significant risk for drops by staff members or accidents where the resident’s body doesn’t move in the direction expected – potentially breaking bones or dislocating joints.

05/25/2017 Failure to give proper treatment to residents with feeding tubes to prevent problems (such as aspiration pneumonia, diarrhea, vomiting, dehydration, metabolic abnormalities, nasal-pharyngeal ulcers) and help restore eating skills, if possible. Caregivers failed to follow proper procedures for residents with feeding tubes in place. A protein supplement with no date or label was observed being administered to a resident. On multiple occasions, the LPNs failed to take the time to check for proper placement for the feeding tube.

06/07/2018 Failure to ensure medication error rates are not 5 percent or greater. Several medication errors were made during the survey period. One resident was administered only ⅓ of their dose of medication (the prescription indicates 4 tablets per dose, only one was administered.)

Another was to receive two tablets of a medication, only received one. Yet another resident was given two cups, one with pills to swallow, another with one to chew. When the LPN was questioned as to which was the chewable, they responded incorrectly, and proceeded to ask the resident to chew a non-chewable tablet.
