Aperion Care Evanston is a small 57 bed nursing and rehabilitation center specializing in post-hospital care and therapy available 7 days per week. They boast amenities such as cozy fireplaces and scheduled activities, a salon, cable TV and wifi “ensuring you have a comfortable healing experience while you recover and prepare to return home”. 09/21/2017 Failure to provide a safe, clean, comfortable and homelike environment. The facility was cited to have a pervasive urine and mildew/dirty clothes smell throughout both care floors. The off-putting smell permeated the facility to the point that the origin couldn’t be identified, as it was everywhere. When staff were questioned about the smell, they wrote it off as something that was just sort of “there” and not of major concern – even though their environment smelled dirty.

09/21/2017 Failure to give residents proper treatment to prevent new bed (pressure) sores or heal existing bed sores. Various breakdowns in communication and improper assessments led to the unnecessary decline in condition of a residents’ pressure ulcer on their ankle.

9/21/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. A resident was injured, requiring hospitalization, due to improper transfer methods moving the resident from a wheelchair to bed. After interview, it appeared that proper transfer procedure was only loosely followed in general.
