Aperion Care Forest Park is a 232-bed nursing home facility in Forest Park, IL. This facility offers short term post-hospital acute care and rehabilitation with physical, occupational, and speech therapies available 7 days a week, with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.”
Amenities include a library, beauty salon, movie theater, ice cream parlor, coffee bar, and transportation services. A quick glance through the Google reviews will generally give you an idea of what to expect from a facility, and in this case, the outlook isn’t good. Most reviewers noted rude, inexperienced staff, not enough staff, and dirty facility conditions.
While these are just opinions, the facts reflect a similar environment. Records indicate that this facility has had 36 complaints that resulted in a health citation in the past three years, including mishandled transfers resulting in broken bones, medication errors, and ignored grooming and hygiene.
04/21/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. The facility failed to follow safe transfer protocols for a resident, resulting in a broken leg. When providing care to the resident after breakfast, a CNA noted and reported that the resident’s right foot was swollen, purplish/red, warm to the touch, and painful. An x-ray and evaluation revealed a fractured tibia.
Further investigation found that a CNA on the previous evening shift (3-11) The surveyor asked the CNA what method was used to transfer the resident. The CNA stated he grabbed the resident under the arms and pivoted him to his bed. The resident’s care plan indicates the use of a sit to stand lift for safe transfers.
07/05/2018 Failure to provide care and assistance to perform activities of daily living for any resident who is unable. Personal hygiene needs were not attended to for all residents that were reviewed for “assistance in activity of daily life” care. Multiple residents even asked the surveyor if they could help them cut their fingernails as they kept snagging on clothes and scratching themselves.
Several other residents, observed with excessively long nails with black material caked underneath, unkempt hair, and facial hair, complained that the facility is always short on help, and no one has time to take care of grooming needs.
07/05/2018 and 06/05/2018 Provide and implement an infection prevention and control program. Two events of poor infection control were observed by the surveyor on separate occasions. When performing wound care to a resident, the resident was noted to have had a bowel movement. The Wound Care Technician put on gloves and cleaned of the resident with a wet towel.
After doing so, the Technician didn’t remove the soiled gloves and proceeded to touch the residents head, bed, hands, pillows, and linens. When asked they stated they weren’t paying attention and should have removed the gloves.
Another instance occurred at a later date. When the surveyor returned, a CNA was found to be feeding a resident who was on an isolation protocol with no personal protective equipment. The CNA stated she wasn’t aware that the resident was still under isolation protocol. There was no sign on the door indicating as such.
Without indication on the door, housekeeping was also unaware. A list of individuals on isolation protocol was provided by the Nurse Manager – this individual was to be on contact isolation for a highly resistant e-coli of the urine.
