Their website boasts a serene setting adjacent to a lush nature preserve, and individualized care plans to meet the residents’ medical, rehabilitation, and personal needs while recovering and preparing to go home.

Our research has found improper skin ulcer care, careless staff, poor hygiene, and dirty, unsanitary conditions:

03/09/2018 Failure to provide care for residents in a way that keeps or builds each resident’s dignity and respect of individuality. A resident was observed sitting in the dining room at lunch time (noon) with wet pants, with visible dark yellow wet area on her inner thighs and a strong urine odor. She was served her food and pushed closer in to the table with no reports to nursing staff regarding her incontinence. She ate her lunch with soaked clothes. Later she was observed in her room (2:30pm) with the same wet pants and same urine smell.

03/09/2018 Failure to 1) Make sure that each resident’s drug regimen is free from unnecessary drugs; 2) Each resident’s entire drug/medication is managed and monitored to achieve highest well-being. Antipsychotic medication was being used on a resident that did not have an evaluation for needing psychoactive medication, even though there was a request on their admitting paperwork to re-evaluate the need for the medication (from a month prior)

03/09/2018 Failure to Make sure that each resident gets a nutritional and well-balanced diet, unless it is not possible to do so. A resident who has a history of significant weight loss has been prescribed supplemental nutrition “magic cups,” but it was observed that during multiple meals, the resident didn’t get the supplement. This resident dropped 12 percent of their weight from January to February – a significant loss.

1/31/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Multiple patients had documentation of pressure ulcers or risk for pressure ulcers. There were specific care instructions listed on file. There was also no record of these treatment plans actually being followed on several occasions.

1/17/2018 Failure to provide and implement an infection prevention and control program. A patient with a known MRSA infected wound was observed getting a dressing change. The nurse treated the wound as any other, with no protection precautions, and didn’t even disinfect the scissors (That had cut through drainage-soaked bandage) before using them to open the fresh dressing package and cut the gauze. MRSA is highly contagious, difficult to treat, and extra precautions should be taken to avoid contamination.
