One resident was observed without proper dressing to a skin tear during incontinence care. Another resident had an unbandaged bleeding scab on one wrist.
5/16/18 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. The facility failed to ensure ordered treatments and interventions were in place for two residents with pressure ulcers. Another resident’s pressure ulcer was observed as being moist, despite physician’s orders to keep it dry in order to heal it.
5/16/18 Failure to develop and implement policies and procedures for flu and pneumonia vaccinations. The facility failed to ensure influenza consents were obtained before administration. Two residents’ face sheets showed no consent of refusal. Two residents received the vaccine without a signed consent. A further two residents received the vaccine after verbal consent, but records do not indicate who gave the consent.

Taking Action
If you observe abuse or neglect of a loved one or possible signs of it, report it immediately. Document what you have seen and take it straight to the facility’s management. They should be willing to hear you out and take immediate action to investigate and fix the issue. However, if they become dismissive or defensive in your conversation or fail to take effective action, it may be time to go to the law.
