Aperion Care Plum Grove is a small, 69 bed short term rehabilitation, skilled nursing, and physical therapy facility in Palatine, IL. They offer physical, occupational, and speech therapies 7 days a week, with personalized care plans “ensuring you have a comfortable healing experience while you recover and prepare to return home.” Other amenities include cable TV, WiFi, daily housekeeping, laundry service, and beauty salon.

This facility is rated “much below average” by medicare.gov, with extremely low staffing numbers as a main factor. Aperion Care Plum Grove residents receive 19 minutes of RN level care per day, less than half that of the Illinois average, and just 3 minutes of physical therapy, half that of the Illinois average. Some of their recent health citations have included:

06/14/2018 Failure to ensure that each resident is free from the use of physical restraints, unless needed for medical treatment. Multiple residents were observed to be under some sort of physical restraint without medical necessity, leading to frustration, agitation, and potential fall risk.

One resident was observed at the end of his bed attempting to get up, and very agitated as he could not. His bed was positioned extremely low as he has a fall history. Firm, 7-inch-high bolsters on each side of his bed were observed. CNA stated they were in place to prevent him from getting out of bed. Two other residents also had these bolsters in place to keep them from getting out of bed. Bolsters or restraints are not in any of their care plans for fall prevention and no assessments were made to see if residents were able to safely exit their beds.

Multiple residents were observed in restraint situations in the dining area. One individual was observed becoming increasingly agitated trying to move out of a space in a wheelchair. He was boxed in by several other residents and chairs, so he could not move from a small enclosed area. He asked a CNA for help getting out, she responded “just a minute” however did not help.

The director of nursing was informed of the situation and defended the CNA, but then agreed the resident was being restrained and moved other residents to free him.

Other residents were observed to be pushed in to the table so far that their wheelchair would be stuck, so it would not move without a staff member pulling them away. This resulted in visibly agitated residents.

06/14/2018 Failure to provide care and assistance to perform activities of daily living for any resident who is unable and Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. The facility failed to assist those who needed assistance with eating, nail care, general grooming, toileting, and positioning. Several residents were left for extended periods of time with no incontinence care or positioning.

One resident with a care plan listing extensive assistance for eating and toileting and reposition every two hours - was observed in her wheelchair, lying in the same position, with no incontinence checks from 10:45am until after 1:45pm.

Another resident was taken from the dining room (where she sat from 8:00am for breakfast) to an activity and returned for lunch with no continence check or care. She was served, but not assisted with lunch. Her lunch sat uneaten as she received no assistance or encouragement to eat. She remained in her chair until 2:00pm when incontinence care was finally provided. An open sore was observed on her buttocks. This wound had developed while in care at the facility, and the resident was not receiving proper wound care.

Several other residents were observed with unkempt, long fingernails, with crusted substances beneath them.

There are some warning signs that you should be aware of that can may alert you to potential abuse or negligence; new bed sores, unexplained bruises, dirty physical appearance, regularly wearing soiled clothing and undergarments, personality changes, weight loss, listlessness and avoidance of regular activities among others.
