ManorCare Naperville is a 115-bed, for-profit nursing home in Naperville, IL. They offer post-hospital rehabilitative care as well as long-term residential care. Their website boasts that ManorCare facilities are “vital medical care and rehabilitation centers with a commitment to specialized care that focuses on individual needs.”
ManorCare of Naperville has had 21 complaints that have resulted in a citation in the past three years. Some of these include insufficient hygiene practices, poor wound care, and poor incontinence care resulting in injury:
09/14/2017 Failure to provide necessary care and services to maintain the highest well being of each resident. Multiple failures in pain assessment, address skin issues, and assist in proper hygiene left a resident with a bright red, raw moisture associated skin disorder on his genitals and inner thighs, leaving him in a painful, burning condition for at least 3 weeks. The resident told the surveyor that the staff doesn’t take him to the bathroom, they just put a “pull up” on and change when he goes to bed at night. He stated he hadn’t been properly showered since he arrived at the facility and can’t clean himself well. Review of records shows he was supposed to have a skin observation audit every Thursday. This record only showed initials of nurses, with no details on observations. A Nurse Practitioner was interviewed in follow up. She confirmed his specific condition was due to poor hygiene, which he relied upon staff to provide.
09/14/2018 Failure to Assist those residents who need total help with eating/drinking, grooming and personal and oral hygiene. Several residents have expressed that they have not been showered in extended amounts of time. Some have gone their entire stay without a shower. One was indicated on the shower record as “refused” when the resident stated that she would not refuse a shower.
07/10/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Treatment orders for pressure ulcers were not followed for 2 out of 3 residents reviewed. One resident was observed to have pressure sore on both left and right buttocks. There were two small open areas on the each as well as one in the tailbone area. The RN cleansed the right and applied a topical treatment. When asked about the areas left untreated, she stated she didn’t see them. The order on record only indicated treatment for that right area. (Indicating the left and tailbone sores may be formed while in the care of the facility, as well as the nurse strictly following orders and not addressing new areas of concern.)
Another resident observed has specific treatment orders, including a common cleansing solution and an anti-stick wound dressing. Neither of these things were found on the care cart. The cleansing solution was located after a half hour hunt, and the dressing was not found at all.
