03/13/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. A resident was observed on multiple occasions to be in need of incontinence care, with her brief completely soaked, and in one instance was soaked through to the sheets. Her files state she needs extensive assistance due to poor mobility and is at high risk for pressure ulcers due to a combination of poor mobility and incontinence. When the surveyor observed the CNAs providing incontinence care, she had extensive bright red and raw areas on her behind. Later she was given dinner when she stated she was uncomfortable and needed incontinence care. The CNA didn’t return. Later, the CNA came to assist, telling the surveyor that she was the only CNA for 29 residents at the moment.

03/13/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. A resident that requires full-time supervision due to confusion and a tendency to fall while trying to get out of bed – in fact, that is why he is at the health care center, rehab after a fall at home. The resident was left alone for a time and then was found sitting on the floor on his bottom. He was sent to the ER after the fall, when his leg was observed to be internally rotated.

12/02/2016 Failure to Store, cook, and serve food in a safe and clean way. Based on observations from a health surveyor, the facility failed to correctly sanitized pots and pans, monitor food temperatures, and maintain clean food prep areas. Dates and temperatures of food were not recorded; in fact, the book to record the data in was missing. Sanitizer was not used at a high enough concentration to be effective. Warming tables were observed to be dirty and not functioning (giving a good reason behind many resident complaints about food temperature!)
