12/14/2017 Failure to provide and implement an infection prevention and control program. An RN was observed to not follow proper infection control protocol while taking glucose readings. She placed the meter in her pocket, also where her clean gloves were, and failed to properly wipe the unit down with a disinfectant towel.
06/12/2018 Failure to ensure medication error rates are not 5 percent or greater. Multiple errors in insulin administration were made during the observation period, as well as a completely arbitrary amount of a topical medicine applied when specific dosing and a dosing card is provided.
