The merely Average health inspections rating comes from instances that caused minimal harm or had the potential for harm to a few residents and include the following events:

4/12/18 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. Staff failed to provide adequate repositioning for one resident which resulted in an avoidable coccyx pressure ulcer.

4/12/18 Failure to provide enough food/fluids to maintain a resident’s health. Staff failed to provide assistance with eating to two residents and neglected to address the weight loss of these and a third resident.

4/12/18 Failure to provide and implement an infection prevention and control program. A staff member failed to put on gloves and protective gown while in a resident’s room who was under contact isolation for a medical condition, increasing the risk of spreading an infection.

Protecting Senior Residents

If your loved one lives in assisted living, they rely on your help to stay safe. You can help ensure they receive the care they need by staying in contact with them and their caregivers and questioning anything that doesn’t seem right. Visit as often as possible to make your own observations and assess how your relative is doing.

Be informed as to signs of elder neglect and abuse. Some of these signs include unexplained injuries or frequent hospitalization, sudden weight loss, severe bedsores, an unkempt appearance, dirty facilities, sudden changes in personality and behavior such as becoming withdrawn or violent, incidents of wandering, and personal belongings being habitually misplaced or lost.

Never be afraid to question protocols or lack thereof. If you have observed signs that your loved one might have suffered mistreated, document what you’ve seen and take it to the facility’s management. If they do not answer your questions or take action to fix the problems, or become defensive and dismissive, you may need to resort to legal action.
