ManorCare of Libertyville is a nursing home with 150 beds and offers care for both short- and long-term residents as well as memory care. Medicare gives ManorCare of Libertyville an impressive overall rating of above average, with a much above average rating for quality of resident care. Staffing and health inspections ratings were found to be average.

Despite these good ratings, there have been a few complaints made at the facility in 2018, such as:

9/20/18 Failure to ensure that a nursing home area is free from accidental hazards and provides adequate supervision to prevent accidents. Two residents with limited mobility experienced multiple falls. One resident reported staff took over 20 minutes to respond to call lights and some staff seemed unaware of her handicap. The second resident experienced multiple falls due to lack of any fall prevention policies at the facility.

5/9/18 Failure to provide care and assistance to perform activities of daily living for any resident who is unable. Seven residents did not receive the assistance required for personal hygiene care for toileting and showers. This left some with soiled incontinence pads and beds and rashes on their bodies. Other residents were found to have greasy hair and generally poor hygiene after missing showers.

4/16/18 Failure to provide appropriate treatment and care according to orders, resident’s preferences and goals. Staff failed to assess a resident before discharging them from their care. A resident’s family member noticed the resident had symptoms of illness and an altered mental state and reported it to staff, who still failed to do a complete assessment of the resident. The resident was re-hospitalized per the family’s request.
