The health inspection reports will tell you exactly what the facility has been cited for in the past so you can compare the number and severity of citations between different homes as well.
Medicare’s information should help you whittle down your choices to a top few. You should visit these facilities to see what they are like for yourself. You can learn a lot from observing the general environment of a nursing home. Pay attention to how clean it is, the friendliness of staff, and the demeanor of residents.
Visiting also allows you to ask questions in person. You probably already have plenty of questions about costs and scheduling, but feel free to ask about anything pertaining to resident safety such as the ratio of staff to residents or what their procedure is for reporting abuse or filing a grievance.

12/06/2018 Failure to protect each resident from all types of abuse such as physical, mental, sexual abuse, physical punishment, and neglect by anybody. The facility failed to protect a resident from physical abuse. The resident sustained a large bruise to his forearm when a CNA jerked his arm while providing hygiene care. He stated she pulled his arm when he was not moving fast enough for her.
12/06/2018 Failure to assist a resident in gaining access to vision and hearing services. The facility failed to ensure hearing aids were maintained and to schedule doctor’s appointments for hearing needs for one resident. The resident was observed with only one hearing aid and demonstrated difficulty hearing. The resident stated that they could afford hearing aids, but the facility held all of their money. The resident’s care plan states to check that hearing aids are in place, with no documentation of the missing second hearing aid. They did not have an order for a new hearing aid nor had the resident seen a doctor about their hearing that year.
