While not every Lexington Health Network facility is operating at an extremely poor level, a number of them are continually struggling to provide even adequate care.
Ignoring the resident’s toilet needs, letting some go for most of a day without being checked on. One resident was observed to have apparently removed their own overfull incontinence brief, return to the bed, and then thoroughly wet the bed. Another had sat in a wheelchair for most of the day, with urine and feces soaking through to the cushion.
Not properly following safe transfer protocols - not providing adequate assistance moving from a wheelchair to toilet or bed, resulting in falls and broken bones.
Leaving patients in pain for over an hour before responding to calls. A resident was observed crying out in pain, waiting for someone to bring her relief. She was left waiting for over and hour before she was given medication to relieve her pain.
Dirty, haphazardly cared for kitchen items, including a dishwasher that wasn’t actually distributing disinfectant, caked on food on pans, and supposedly clean meat slicer that had meat and other “multicolor” particles in it.
Irresponsible care of patients with contagious infections such as MRSA, Not changing gloves after assisting a MRSA infected resident use to bathroom, not properly washing hands in preparation and after taking care of wounds in the tailbone area.
Multiple problems due to inadequate staffing, including a staff member resorting to locking down several residents in the dining hall as she couldn’t provide assistance for them all while alone, several with toilet needs all at once.
A fall that resulted in broken legs, due to negligent supervision of a known fall-risk resident. This resident unfortunately passed away due to complications from the fractures.
