Lexington of Orland Park is a large, 259 bed “for profit” health care center in Orland Park, IL. They offer multiple types of therapies and rehabilitative services, including stroke rehab, physical, occupational, respiratory, and speech therapy, wound management, complex medical care, and post acute care.

Their web advertising says they are “passionate about offering superior health care and senior living options.” Recent health inspections show poor incontinence care, poor measures for infection control, poor resident hygiene care, poor kitchen cleanliness protocols, and more;

06/09/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. A resident was injured (two broken bones in the foot and leg) when nursing staff failed to use the proper equipment for transfer. The resident was being assisted in the shower, and finishing up. The CNA allowed the resident to attempt getting herself into the wheelchair, which was not the protocol in place. (She was to be transferred using a stand to sit assisting device.) The resident didn’t fully turn to sit, and fell, breaking her foot and femur.

06/09/2018 Failure to make sure that each resident who enters the nursing home without a catheter is not given a catheter, and receive proper services to prevent urinary tract infections and restore normal bladder function. A resident was observed at nearly noon, in need of incontinence care. She stated she hadn’t been changed yet that day, and she woke at 10. At 3pm, she was taken care of, with wet brief and bowel movement. The CNA didn’t have a record of when the resident was last changed.

04/12/2018 Failure to provide and implement an infection prevention and control program. Multiple instances of deficient infection prevention were witness during the inspection. Improper hand washing, improper disinfection of blood glucose monitoring devices, and allowing oxygen lines to lay on the floor.

04/12/2018 Failure to provide care and assistance to perform activities of daily living for any resident who is unable. Several residents were observed to have long, overgrown fingernails, with substances caked under the nails. Others were seen with greasy, unwashed looking hair and scruffy, unkempt facial hair. These things should be cared for by staff under Activity of Daily Living care.

04/12/2018 Failure to procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards. Failures in proper food safety and upkeep of sanitary conditions as observed have the potential to cause food borne illnesses. Food was stored improperly, appliances not clean with food particles and standing water in them. Basic sanitation and food temperature recording procedures were mostly news to the kitchen staff, as if they were never trained on doing these tasks.
