Lexington of Streamwood is a 214 bed “for profit” nursing home in Streamwood, IL. They offer private and semi-private rooms and bathroom, wound management and post-acute care, including dialysis, full rehabilitative services including physical, occupational, and speech therapy, and therapeutic modalities. Their website boasts that they are “passionate about offering superior healthcare and senior living options,” but unfortunately, their public records speak a different story;
05/02/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. A resident that is completely dependent on staff for care was severely injured in a fall when CNAs failed to use the correct equipment to transfer her from her bed to a wheelchair. Her care information clearly lists the correct transfer information, including a note to be aware that she has poor trunk control and a known behavior of leaving forward. The CNAs used a sling lift of the wrong size that allowed her to lean forward and fall on the floor during the transfer. She was taken to the hospital with bleeding from her head, later diagnosed with a brain bleed from the fall.
12/26/2017 Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. A resident who was listed as a fall risk, who requires assistance while drinking, was left unattended in her wheelchair, spilled her juice, and in an attempt to pick up her cup, fell out of her wheelchair. This fall resulted in breaking both of her femurs. She passed away with her death certificate listing complications from bilateral femur fractures.
12/26/2017 Failure to protect each resident from all types of abuse such as physical, mental, sexual abuse, physical punishment, and neglect by anybody. A resident was verbally assaulted by a CNA while she was to be having a bed bath. The CNA was heard cursing, saying “F…you!” to the resident. The altercation was heard from the hallway, and the CNA was removed from the situation. (She was terminated after investigation.)
12/26/2017 Protect each resident from separation (from other residents, his/her room, or confinement to his/her room). This incident occurred as a direct result of understaffing. A single CNA was left in charge of approximately 17-20 residents at the dining room, and everyone wanted to leave or to use the bathroom all at once. She ended up locking down residents’ wheelchairs and in a panic, closed the doors to the dining room so no one could leave. One resident was told outright that she had to stay where she was and couldn’t leave. Both residents and the CNA were panicked. Another Aid overheard yelling from the dining room and came to see the situation, and assisted in calming residents.
