Lexington of Lombard is a 215 bed for-profit nursing home facility in Lombard, IL. They offer multiple types of therapies and rehabilitative services, wound care, complex medical care, and post acute care. Their advertising says they are “passionate about offering superior health care and senior living options.” Health inspections tell a less-than-superior story. In the past 3 years, there have been a total of 18 health complaints that have resulted in a citation.

08/17/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. A resident was severely injured in a fall when a CNA ignored transfer protocols and the residents statement that his knee hurt. Another individual was listed as high fall risk and was to have an alarm system in place to alert staff of falls. This system was not working. The resident had 5 falls listed in their records.

08/17/2018 Failure to provide food in a way that meets a resident's needs. Many residents in nursing home care eat specially prepared foods of specific consistencies. Pureed food is meant to be a pudding consistency. Multiple residents were served thick, dry, peanut butter consistency purees of rice and pastas, and grainy, unpalatable meat and fish. This is a dangerous aspiration and choking hazard.

08/17/2018 Failure to have a program that investigates, controls and keeps infection from spreading. Multiple instances of improper disinfection were observed, along with careless treatment in regards to a resident with clostridium difficile (A severe intestinal infection that can be transferred by contact).

11/16/2017 Failure to make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents. A resident fell, resulting in multiple fractures to her lumbar spine. This fall was a direct result of negligent use of the mechanical lift, designed to help safely transfer patients.
