Breaking Down the Patient Care Plan Process

The assessment information should be discussed in a meeting with family members, staff, and medical professionals and be used to develop a care plan that will serve as a guide to discuss the resident’s needs and progress outlined in a timeframe. In 2017, the Centers for Medicare and Medicaid Services (CMS) implemented new changes to the basic requirements that must be met by nursing homes in order to receive funding. The new rules say, “the baseline care plan should strike a balance between conditions and risks affecting the resident’s health and safety, and what is important to him or her, within the limitations of the plan’s timeframe.”

Symphony of Buffalo Grove Staff Ignore Patient Care Plans

Symphony of Buffalo Grove is a 200-bed recovery home located in suburban Chicago. One of the for-profit rehabilitation facility’s recent lawsuits was for the alleged neglect and under supervision of a patient’s care plan that led to the cruel and untimely death of the 81-year-old woman. The deceased was admitted to the facility in June 2016 and over the course of just 15 months became dehydrated and malnourished on several occasions, suffered multiple unoccupied falls, and developed painful, untreated pressure ulcers due to failure of being properly cared for.

The goal for all residents of rehabilitation facilities should be to get better, not worse. When a patient is admitted, a care plan should first be established to ensure that detailed information will be followed for staff to help maintain or improve a patient’s physical and mental well-being. In addition, fall risk determination and necessary fall prevention methods should be acknowledged, a detailed list of current medications given, and a strict nutritional guide established. Trends in patient needs should be evaluated over time and reflect any care plan changes.
