Symphony of Orchard Valley is a large, 203-bed nursing home facility in Aurora, IL. They specialize in post-acute care and pride themselves on quick recoveries with optimal results. Their website advertises a “fine hotel,” tranquil setting, “ensuring the utmost comfort of our residents, whether they live here or are here to recover and get back home.”

Symphony of Orchard Valley has a below average staffing rating with Medicare, indicating that the number of staff to resident ratio may not always allow for adequate care. This facility has had an alarming 34 complaints that have resulted in health department citations in the past 3 years. Some of these citations have included falls with injuries, extremely slow response to call lights to address resident needs, and overuse of physical restraints.

12/14/2017 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. Nursing staff failed to stay in the shower area while a resident who was a high risk for falls was in a shower chair. The resident reached for the soap and fell out of the bathing chair. This fall resulted in a broken leg as well as a bruise to the forehead, and later a stroke.

09/27/2017 Failure to provide necessary care and services to maintain the highest well being of each resident. In this instance, a resident had complained of pain due to an open pressure wound on the coccyx and lower back. They state the pain was 8 out of 10 on the pain scale. When the dressing was being changed and the area treated, the resident was observed flinching away repeatedly, and crying out in pain. Pain management policies were clearly not followed.

09/27/2017 Failure to assist those residents who need total help with eating/drinking, grooming and personal and oral hygiene. Several residents had complained that they hadn’t been getting their showers as scheduled. Records reflected the same, some residents receiving only half of the bathing care they had been scheduled for, leaving some with showers only happening a few times a month.
