The facility was found to have failed to follow their clothing list policy and record items brought into the facility. They also failed to follow their missing items policy by not promptly investigating a report of missing items for one resident. The resident stated that their personal clothing was missing, and at the time of the report no investigation into the missing items had been made.

9/7/18 Failure to procure food from sources approved or considered satisfactory and store, prepare, distribute, and serve food in accordance with professional standards. Staff failed to ensure a blender air dried before use, with the potential to affect 58 residents on soft and pureed diets. Staff also failed to discard of food after use by dates, and a dietary aide was observed as not wearing a facial hair covering when handling food.

2/16/18 Failure to honor the resident’s right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely. It was reported that staff laid a clean pad on top of a resident’s dirty fitted sheet three times instead of changing the soiled sheet.
