6/8/18 Failure to provide pharmaceutical services to meet the needs of each resident and employ or obtain the services of a licensed pharmacist. The facility was found to have failed to follow their policy in documenting reconciled controlled and narcotic substances during each shift and storing Schedule II controlled medications under proper security. The facility also failed to follow their policy in documentation of medications administered to a resident.

6/8/18 Failure to provide or get specialized rehabilitative services as required for a resident. Staff failed to evaluate a resident’s swallowing abilities and to recommend a proper diet upon admission. A resident was readmitted with doctor’s orders for a pureed diet but was instead given normal foods based on an assessment from a previous admittance.

How to Prevent Elder Abuse in Assisted Living
