1/12/18 Failure to honor the resident’s right to a dignified existence, self-determination, communication, and to exercise his or her rights. Staff failed to provide care in a dignified manner to four residents. One resident was left lying in an uncomfortable position and unmade bed when the employee helping them was called away and failed to notify anyone that they still needed help. One resident complained of a staff member throwing items to them. One resident found personal belongings rearranged in a way that made it impossible to reach them. Another resident reported a staff member responding to a request under her breath in a foreign language.

1/12/18 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. The facility failed to provide regular two-hour repositioning for a resident with stage 4 pressure ulcers, putting the resident at risk for more severe ulcers.

1/12/18 Failure to ensure each resident receives and the facility provides food prepared in a form designed to meet individual needs. The facility failed to provide properly pureed foods for seven residents, putting them at risk of choking.

How to Protect Assisted Living Residents
