6/14/18 Failure to provide appropriate care for a resident to maintain and/or improve range of motion, limited range of motion and/or mobility, unless a decline is for a medical reason. The facility failed to assess four residents with contractures and implement interventions such as providing splints or other devices.

6/14/18 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. The facility failed to ensure staff were knowledgeable in how to transfer residents with hip precautions and failed to ensure a safe transfer by not using a mechanical stand lift and a gait belt.

One resident experienced pain when a staff member helped her into bed incorrectly. It was later found that her hip replacement was twisted, setting back her recovery. A second resident was pulled into a standing position by the forearms against instructions to use a mechanical stand. Finally, staff failed to offer a resident a gait belt when the resident moved from a wheelchair to the bathroom.

How to Protect Your Loved One in Assisted Living
