Alden Valley Ridge Short Term Rehabilitation and Health Care Center is a 207-bed for-profit rehabilitative, post-acute and short-term health care facility. Alden’s advertising gives the impression of an upscale, “5 star” experience, framing themselves as something beyond your standard nursing home. Their website boasts a “holistic approach to quality care, using innovative therapies designed to send patients home as quickly and safely as possible.”
“Luxurious accommodations” and “indulgent amenities” are intended to make Alden an enjoyable place to call home while rehabilitating from surgery or recuperating from a hospital stay. While many Alden facilities are highly rated by Medicare and have very few care violations on record, there are always exceptions. Alden Valley Ridge is one of these exceptions, with 24 complaints resulting in a citation in 3 years and a below average rating with Medicare.gov. Here’s just a few examples of the citations they’ve received:
7/20/2018 Failure to provide appropriate pressure ulcer care and prevent new ulcers from developing. The facility failed to properly provide toileting care and repositioning for a resident who requires extensive assistance, resulting in the development of stage 3 and 2 pressure sores. Family reported that they have visited regularly from 10:00am to 7:00pm and he was never repositioned.
Even after sores had developed, the surveyor observed that he was not being repositioned or checked for continence care needs (observed from 9:45a-1:30p). The pressure sores were documented as a stage 3 1.2cm x 1.5cm, and a stage 2 2cm x 2cm. His physician stated these ulcers could be easily prevented.
7/20/2018 Failure to ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents. Multiple residents received injuries due to falls. One resident who requires assistance in transferring and ambulation around rooms suffered a fracture to her hip after falling while trying to close draperies. No incident report was filed nor investigation of the fall.
Another resident had an unwitnessed fall resulting in a fractured wrist. Yet another high risk for fall resident was put to bed with compression hose on (Without non-skid slippers over.) Upon the next check, he was found on the floor, on his back, unresponsive. He suffered a subdural hematoma, is currently on life support with no power of attorney to make end of life decisions.
7/20/2018 Failure to provide enough nursing staff every day to meet the needs of every resident; and have a licensed nurse in charge on each shift. Based on observations and interviews, the facility failed to have sufficient staff available to properly assist residents in daily life needs and did not answer call lights in a timely manner. Several residents were observed to have developed pressure sore and ulcers due to lack of attentive care in repositioning and continence checks.
Multiple residents were observed needing assistance in feeding themselves and struggling, with no assistance. Residents that are continent and can make it to the bathroom with assistance reported that they were told to just go in their incontinence brief. Other complaints include residents not getting showers, not getting grooming assistance, call lights going unanswered for an hour or more. CNAs stated that there just isn’t enough staff to provide good care.
